Sam hums softly. "Not right now," he says after a moment. "Not that I don't want your mouth, but I'd rather see to you, first. Maybe tonight, though." He isn't going to rule it out, after all. "But I think you could definitely keep me warm if we decide to take a nap after the cream is on."
Which...Sam is all for, honestly. His sleep was a bit broken, wanting to be careful of Castiel and make sure he hadn't hurt him too much. So a nap, now that he knows Castiel is all right, would be good.
