There's something in that last bit that makes Sam's blood run cold. "Guests?" he asks, keeping his voice very tightly controlled as he strokes his fingers down Castiel's arm. "What do you mean by guests?"

Castiel obviously wasn't in any condition to deal with guests of any kind. So the fact that there were some expected was probably not a good thing.
